Please note that an 8.3125% sales tax will be added to all services as required by the State of New Mexico.
*Service rates for on-site acupuncture and home care personal treatments may vary depending on the number of people, session duration, and half or full-day availability. Thus, patients must book these services directly with me.
Cancellation Fee
Patients who cancel their service appointment less than 48 hours in advance will be charged a cancellation fee of $175.00
Accepted Insurance Information
As an in-network participating provider with UnitedHealthcare and Aetna, I accept payments through these two insurance companies, provided my services are eligible for reimbursement by a patient’s insurance plan. Payments through other insurance carriers may also be accepted if your plan covers out-of-network providers, or an itemized bills can be provided for you to submit directly.
Insurance verification forms must be completed and your plan must be verified by your insurance provider prior to scheduling a new patient appointment. Please do not hesitate to reach out to me today to ask if your insurance plan covers my services.
Cancellation Policy
Thank you for choosing RareBird Acupuncture. As a courtesy to all of my patients, I strive to maintain a smooth and efficient operation so that I may provide the best care possible. If you need to cancel your appointment, please do so within 48 business hours or a cancellation fee of $175.00 will be collected.
If you are late for your appointment, your time will be shortened to accommodate on-time patients. Patients who are more than 20 minutes late will be considered a no-show and charged the full cancellation fee. I respect your time and ask that you do the same. Your cooperation and consideration are greatly appreciated.
COVID-19 Clinic Policy in Effect
RareBird Acupuncture is a healthcare facility, and our COVID-19 clinic policy remains in effect.
Masks are required for the duration of your appointment. Chinese medicine excels at treating illnesses. However, if you are actively sick or starting to show signs of illness, you cannot come into the office for treatment and will have to reschedule or do a telemedicine appointment. If you have traveled or have recently had close contact with anyone who has traveled, you must wait 3 to 4 days before coming in for an appointment. If you have COVID please wait until you test negative to return for treatments. Thank you.
